What Is Lichen Planus? Understanding The Skin Condition

Lichen Planus is an inflammatory skin condition that causes a distinctive purple colored rash. Its exact cause is unknown, but it is believed to be an autoimmune reaction where the immune system mistakenly attacks healthy skin cells. Some potential triggers that may cause the immune system to malfunction include:

– Medications: Certain drugs like diuretics, beta-blockers, and antimalarial medications have been linked to Lichenoid keratoses in some cases.

– Hepatitis C Infection: There appears to be a link between Lichen Planus Lichenoid keratoses and hepatitis C virus infections in some individuals. The rash may clear up if the hepatitis C is successfully treated.

– Stress: High levels of stress and anxiety may contribute to flare-ups in those with Lichenoid keratoses.

– Genetics: Family history of autoimmune disorders increases the risk of developing Lichenoid keratoses, suggesting a genetic component.

Unfortunately, identifying the root cause is not always possible as Lichenoid keratoses can develop even without an obvious trigger in some people. Multiple factors likely interact to spark an immune reaction in the skin.

Common Symptoms Of Lichen Planus

The classic appearance of Lichenoid keratoses includes itchy, purplish flat-topped bumps (papules) or plaques on the wrists, legs, lower back and genital regions. Other symptoms include:

– Violaceous colored, flat-topped bumps or firm, shiny bumps/plaques ranging from 1-10mm in size.

– Waxy, white lesions inside the mouth (oral Lichenoid keratoses) which can cause soreness.

– Fingernails or toenails that appear ridged or grooved.

– Itchiness or mild irritation associated with the skin bumps or plaques.

– Lesions rarely occur on the scalp or face.

– Symptoms tend to come and go, often worsening during periods of stress.

The rash may initially develop on the wrists or ankles and then spread to other areas over time. Oral and genital Lichenoid keratoses can cause significant discomfort eating or having sexual relations.

Diagnosis And Treatment Options

A dermatologist can usually diagnose Lichenoid keratoses based solely on the appearance of the rash. However, a skin biopsy may be performed to confirm the diagnosis, especially if the rash is atypical or involves internal organs.

Mild cases of Lichen Planus on the skin surface often do not require treatment and will clear up on their own within several months. However, the following options are available depending on severity and location of symptoms:

– Topical corticosteroid creams: Application of mild-strength corticosteroid creams 2-3 times per day can help reduce inflammation and itchiness.

– Oral corticosteroids: Stronger corticosteroid tablets may be prescribed for severe, widespread disease that does not respond to topical treatment alone. This involves a short course of daily pills.

– Topical calcineurin inhibitors: Anti-inflammatory ointments containing tacrolimus or pimecrolimus can provide relief for facial, genital or intertriginous Lichenoid keratoses.

– Antiviral medication: Drugs like Valacyclovir are sometimes used if Lichenoid keratoses is linked to a hepatitis C infection.

– Pain relievers: Over-the-counter analgesics can help reduce irritation from oral lesions. Prescription mouthwashes may also be recommended.

– Laser therapy: A pulsed dye laser treatment option exists for resistant skin lesions.

Prognosis

Most cases of Lichen Planus resolve within a year without scarring. However, in rare cases the rash can persist longer term or recur periodically. Oral Lichenoid keratoses may cause some mouth discomfort long-term in a minority of patients. Prompt treatment provides the best chance of clearing lesions and preventing complications like infection or scarring. Maintaining low stress levels and avoiding potential triggers can also help control this chronic skin condition.
